Persons wishing to file a complaint against an occupational therapist should file a complaint form with the Board. For assistance in filing and determining if the Board has jurisdiction, you may want to contact the Board office at (602) 589-8352.  The board by statue accepts complaints, investigates, and takes any needed disciplinary action against occupational therapists for which it has jurisdiction. 

Generally the Board has jurisdiction over Arizona licensed occupational therapists and persons who hold themselves out as occupational therapists in Arizona.
