Consumer Information

All occupational therapists must be licensed in Arizona and must meet the requirements of statute and rule.  Anyone receiving the services of an occupational therapist should contact the Board to determine if the therapist is licensed and the license is in good standing.

Persons wishing to file a complaint against an occupational therapist should file a complaint form with the Board. For assistance in filing a complaint and determining if the Board has jurisdiction you may want contact the Board office at (602) 589-8352.  The board by statue accepts complaints, investigates, and takes any needed disciplinary action against occupational therapists for which it has jurisdiction.
